We schedule everyone in the order that we receive requests. Please reserve 15-20 minutes to gather all the information needed. You can speed that process up by printing the new patient paperwork, completing it, and faxing it to our office at 985-625-0022.

 

If you are seeking psychiatric services, or medication management, you will also need to get a referral from your primary care physician sent to us. This referral is needed prior to your appointment.

 

If you choose to fill out new patient paperwork at our office, please arrive 15 minutes early to do so. We strive to minimize wait time so that no one is inconvenienced. Arriving past your appointment time may result in having to reschedule your appointment.
